This was developed for a middle school band program that was NOT graded. This was our mid-year parent check in form to communicate student progress. You could swap out the A=Above & Beyond; Q=Quality Work; G=Good; N=Needs Improvement U=Unsatisfactory letters for whatever terms you may use at your school. We avoided ‘Excellent’, and made sure that the top 3 ratings were considered degrees of proficient. This had no bearing on students academic record, just an internal document to offer feedback to students and parents.
